What is Community Health Infrastructure?
Community health infrastructure is the network of people, organizations, relationships, technology, resources, and processes that enables communities to access care and remain connected to it.
Community Access & Navigation
This is the people-facing infrastructure: trusted community entry points, outreach, resource navigation, referrals, care coordination, and ongoing support.
Connected Community Health Networks
This is the system-facing infrastructure: connecting community-based organizations, healthcare providers, public-health agencies, resources, data, and funding into a functioning network.

Community Power & Leadership
Healthy communities are built by people who have the knowledge, resources, and power to shape what happens around them. We invest in community leadership by creating opportunities for people to build skills, deepen their understanding of health and systems, develop solutions, and turn lived experience into action. Through leadership development, education, advocacy, and entrepreneurship, we help strengthen the people and ideas that move communities forward.
Stability & Resource Navigation
Health is shaped by much more than what happens in a doctor's office. Housing, income, food, transportation, benefits, and access to supportive resources all influence a person's ability to thrive. We help people navigate fragmented systems, overcome barriers, and connect to resources that create greater stability—while building stronger pathways between communities and the organizations designed to support them.
Digital Evaluation & Storytelling
Community experience is valuable evidence. We combine storytelling, digital tools, and community-centered evaluation to better understand how people experience programs, services, and systems. By capturing stories, identifying patterns, and translating lived experience into actionable knowledge, we help communities demonstrate impact, organizations learn what works, and systems make more informed and responsive decisions.
